Okay, if you have followed all the steps EXACTLY, from this thread:

If your UI still does not load... The problem is most likely that you first copied the "Default" folder in the UI/ directory, renamed it and THEN proceeded to unzip the UI items you wanted into that new folder. This will cause the UI to no load.
What you need to do is, create a new folder *
WITH NO FILES IN IT* and unzip the UI items you want into this folder.
DO NOT COPY THE FILES FROM THE "DEFAULT" FOLDER INTO THE NEWLY CREATED FOLDER.
