If you extracted the zip to the Everquest II directory, it should have created a file named "eq2.ini". Open that file with Notepad and confirm the last line reads...
cl_ui_skinname FetishNightfall
You should also have the following directory
Everquest II/UI/FetishNightfall
which should also have subdirectories of
eq2maps
images
infocenter
layouts
When you launch the eq2maps updater, it should list "FetishNightfall" as your UI.
